Pass a clear criminal background check. Obtain medical and dental clearance. Provide documentary evidence of current immunizations, medical insurance (illness and injury), Health Care Provider CPR. Possess a current unencumbered, active Certified Nursing Assistant (CNA) license or proof of completion of a CNA Skills Set training. Possess current liability insurance coverage in the amount of $1 million for the LPN/LVN student. Provide transcripts showing completion (3.0 or above) of five quarter-credits or three semester-credits each of (1) college-level math, (2) English composition, (3) human relations, humanities or social sciences, and (4) Anatomy/Physiology-Biology 175 (must include lab) or higher. Provide a Certificate of Completion for HIV/AIDS training (minimum of seven hours).Is this program approved to train veterans?
